Standard enrolment questions example form

Description

The sample enrolment form is designed to assist registered training organisations (RTOs) collect and submit data compliant with AVETMISS for the National VET Provider Collection as required by the Data Provision Requirements 2020 and National VET Data Policy.

How to use this form

Under the National VET Data Policy, all RTOs must implement policies and procedures to collect data compliant with AVETMISS data from their students that is accurate and complete, unless exempt under Clause 5 of the Policy.

This form contains the minimum questions students must be asked when enrolling in order to collect data compliant with AVETMISS. These questions can be incorporated into physical or online enrolment forms for enrolling students.

We recommend the sequence and wording of questions are maintained. RTOs can also add their own questions to their enrolment forms.

It is not compulsory for a student to answer all the questions on an enrolment form. Whilst an RTO should never force a student to answer all the questions, they should not prompt students to select not-specified responses or leave questions unanswered, as this may put the RTO at risk of not meeting the policy requirement outlined above.

Former students of an RTO who return to enrol in additional training must complete the enrolment form again to ensure that their information is accurate and up to date. These students must also be provided with a privacy notice so they are aware of the purposes for which their information may be collected, used or disclosed. It is also recommended that RTOs have a process to ensure that continuing students' information is kept up-to-date to ensure that the AVETMISS data is as accurate as possible at the time of reporting.

In addition to the minimum AVETMISS questions, the sample enrolment form also includes:

  • the name of the AVETMISS Data element (in italics below each question) to which the question maps. This has been provided to assist RTOs and student management system providers but does not have to be used on actual enrolment forms.
  • the minimum mandatory content for inclusion in a privacy notice so that students are aware of the purposes of which their personal information may be collected, used and disclosed. As per Clause 7 of the National VET Data Policy, it is the responsibility of RTOs to ensure students who provide an RTO with personal information that will be included in the National VET Provider Collection are reasonably aware the information may be collected, used or disclosed.
  • a Disability Supplement to assist students with answering Question 11 on the sample enrolment form. This information should be available to students when they are completing their enrolment form.
